Survivors include his mother Nicole Patrick; grandparents Terry and Terry Hurst, Linda and Randall Patrick; great-grandmothers Mary Patrick and Aderinne Kennedy; aunts and uncles Randall & Kristina Patrick, Scott and Noell Richmond, Danny and Amber Burst and Heather Mcheon.

A visitation will be held at the Shirley Brothers Funeral Home on Thompson Road Friday September 3, 2004 from 11 to 1 p.m. Memorial contributions to the St. Vincent's Children's Rehabilitation Hospital at 1707 W. 86th Street, Indianapolis, IN 46260. Church & Community is handling the arrangements.
